Halton Coordinated Service Planning

Coordinated Service Planning (CSP) Halton Coordinated Service Planning (CSP) is part of the Ontario Complex Special Needs Strategy, which was developed to support a higher level of service than Level One Service Coordination, in the Halton community. While ROCK is the Lead Coordination Agency, community partners across Halton Region form the Halton Coordinated Service Planning Collaborative to serve families across Halton.

CSP provides children and youth with multiple and/or complex special needs, and their families with a high level of seamless and family-centred care. The Coordinated Service Planner is directly responsible for planning and coordinating child services, in collaboration with the family and other services and supports. CSP strives to integrate service delivery and decrease stress associated with navigating and coordinating services. Circles of support are created, as needed, around each child or youth and their family to promote healthy child development and to ensure children and youth have access to effective and integrated formal services and natural supports.
